Becoming a patient with us is a very easy process, however, we do need to make sure that you are a candidate for our treatments prior to your admission, the first step towards making sure that you will get the best results possible is to fill out our medical questionnaire which you can find here:

This information will be used by our medical team to make their initial treatment plan recommendations, which will be ready within 48, this initial treatment plan may be modified after your phone consultation if one is scheduled.

After you have submitted your medical questionnaire one of our admissions officers will contact you as soon as possible, using your preferred contact method, to answer any questions that you may have in regards to our clinic, treatments, medical staff or any question in regards to our services.

The following step is to set up a consultation for you to speak with one of our doctors who will design a personalized treatment plan specifically for you and answer any medical questions for you.

Once you have a treatment plan you are all set to schedule your admission and begin your journey to become healthy again!

ADMISSION

Once you have a treatment all you need to do is send us an email or call to let us know the date that you wish to be admitted.

Prior to your admission we recommend that you review your treatment plan, sign it and send it back to us, this can be signed digitally on your phone or computer, or you can print it out, sign it and send us a picture of it, so that we can make a reservation for you. It is important that you review the financial section of the treatment plan, a deposit will be required for you to begin your treatment and/or receive medical attention on arrival, if needed. This deposit can be done prior to your arrival or on arrival at our facilities.
